Obtaining Death Records
Death Records are public information.
In Person
Fill out the application for a Death Certificate by visiting the County Clerk's Office.
By Mail
Complete the Death Record Request Form (PDF) or send a letter that includes the following:
- Name of Decedent
- Date of Death
Your death record request form or written request should be submitted with payment. Checks and money orders should be made payable to Manistee County Clerk. Please include a self addressed business sized stamped envelope and send request to:

Send a written request by email, including the following:
- Name of Decedent
- Date of Death
Death records may be paid for online once email request is sent.
Record Costs
- $15 for the first copy
- $5 for each additional copy purchase at the same time and on the same day

- Check or Money Order made payable to Manistee County Clerk
- Online
